My conjecture is that it all comes down to linguistics and our ages (30 & 81). The English language (as always) is changing and the two of us speak very slightly different versions of it. "Manhole cover" is a part of the language for both of us and would sound pretty forced if we replaced it (not that I wouldn't -- I'm all about proactively changing the language).
